       INSTRUCTIONS TO POLICYHOLDER
 In case you fail to notify your Agent of a change in your address,
please communicate with the District Office or the Home Office in
Mew York, N. Y., or the Pacific Coast Head Office in San Francisco,
Calif.
 When communicating with any office of the Company, always
give the policy number, the name of the insured as entered on the
opposite page, the name and address of the District Office, and the
name or debit number of the Agent who last collected premiums.

condition that any check or draft remitted in settlement may be
handled for collection in accordance with^the practice of the collect-

                 CHANGE OF ADDRESS
 Notify your Agent of any change of address or send your com-
plete new address at once to the District Office, to the Home Office
at 1 Madison Avenue, New York, N. Y., or to the Pacific Coast
Head Office at 600 Stockton Street, San Francisco, Calif.
                   READ YOUR POLICY
 It states exactly what you are paying for. No Agent can change
the terms of the policy in any way. If it is not satisfactory, you
can surrender it within two weeks after the date of the policy and
premiums paid will be returned.
                PAYMENT OF PREMIUMS
 Premiums are due each Monday, in advance. If the Agent does
not collect the premiums when due, send them to the District
Office to the Home Office in New York, N. Y., or to the Head
Office in San Francisco, Calif.  Checks or money orders should be
made payable only to the Metropolitan Life Insurance Company.
 Premiums must be entered in this Premium Receipt Book by an
Authorized Agent, or they will not be credited by the Company.
 Agents are prohibited from paying or advancing premiums for
the policyholder.
 A policy on which premiums are unpaid for more than lour
weeks is lapsed. Be safe. Pay premiums in advance.
     NONFORFEITURE VALVE IN CASE OF LAPSE
 In event of lapse, policies issued since January 1, 1907, upon
which not less than three years' premiums have been paid (not less
than 26 weeks' premiums for policies issued after Dec. 31, 1936)
provide either for Paid-up insurance for a reduced amount, or for
Extended Term insurance, dependent upon the policy conditions
and the laws of the State in which the policy was issued. On policies
issued before 1907, premiums must have been paid at least five
years in order to obtain any Paid-up insurance.  Full particulars
may be obtained at the District Office, or by writing to the Home
Office or to the Pacific Coast Head Office.
        REINSTATEMENT OF LAPSED POLICIES
  If your policy has lapsed, and you are still insurable, it may be
reinstated, subject to the Company's rules. Full information may
be obtained from any Agent or at the District Office.
                         DIVIDENDS
  The Metropolitan is a mutual organization. Its assets are held
tor the benefit of its policyholders, and any divisible surplus is
returned to its policyholders in the form of dividends. The surplus
accruing on Industrial policies is not sufficient to allow dividends
until the policies have been several years in force. These dividends
are announced the first of each year, and full particulars can be
obtained from the Agent or at the District Office.
              LOSS OF EYESIGHT OR LIMBS
  Industrial policies issued after January 1, 1935, provide that if,
while premiums are not in default beyond the grace period, the
insured, as a result of disease contracted after or injury sustained
after issue of the policy, loses by severance both hands, or both feet,
or one hand and one foot, or becomes totally and irrecoverably
blind and survives such loss of sight for 30 days, the Company will,
on receipt of proof of such loss, pay one-half the death benefit and
continue the insurance in force without payment of future premiums.
A similar provision, with some differences in terms and conditions,
is contained in or is applicable to all Industrial policies previously
issued by the Company, but no such benefit is allowable for any loss
that occurred before March 6, 1916. Reference should be made to
the policy or the announcements of the Company for the exact
terms and conditions of the applicable provision.
                    DEATH CLAIMS
Should the person insured die, the claimant should call at or
tify the District Office.  Deliver the policy only to the Company's
aresentative. The intervention of a third person for the collection
the claim or the payment of commission to any person for services
regard thereto is entirely unnecessary. The Company is glad to
y, and there is no need for alleged influence.  Payment will be
ide promptly upon satisfactory proof of death.
EMPLOYEES OF THE COMPANY ARE PROHIBITED

           ACCIDENTAL DEATH BENEFIT
Under Industrial premium-paying policies, if an accident causes
ath within 90 days, the insured being between ages 15 and 69, in-

ms due under the policy, a sum equal to the amount of insurance
en payable at death. The Accidental Death Benefit is reduced
one-half in the case of certain railroad and mining occupations,
d there are other exceptions relating to suicide, aviation, etc.
     PRIVILEGE OF VOTING FOB DIRECTORS
An election of Directors of the Company is held in New York on
e second Tuesday in April of every odd-numbered year.  After
e year from the date of issue of any policy referred to in this

force, will have a right to vote either in person or by proxy or by
